How are the ratings for television shows counted?

Like for shows that air during prime time... How are the ratings counted? Is the Nielsen Family thing still how they do it? Or has that changed?

1. Yes, Nielsens still goes door to door even to get a sampling. I actually had one come by like a month ago. They paid me and gave me a gift basket.
